Important! The application does not replace filing a housing application with University Housing and Residence Life. You must complete a housing application and contract for final placement into the Science Success Fast Track Living-Learning Community. Your application will be reviewed and confirmation of placement into the program will be by July 15, 2009.

Please read and check the box next to each of the following statements. Checking the box indicates that you have read, understand, and agree to the conditions stated.

	I understand that I will be registered by the Office of Learning Communities and the Office of the Registrar for the courses that are part of the Science Success Fast Track program.
	A specific number of seats are held in designated courses for AIMS. I understand that if I choose to drop any of the courses that I was registered for as a part of my learning community that my eligibility for the Science Success Fast Track program can be affected. Status in the program will be determined at the discretion of the Director of Learning Communities and the Science Success Fast Track Program Coordinator.
	To enroll in a residential learning community I must have a completed housing contract for the 2009-2010 Academic Year. I know that I will receive a room assignment from the Housing Office in the residential area reserved for the Science Success Fast Track program.
